Circumstantial Report Template

A circumstantial report template is a tool which is used to prepare a circumstantial report whenever any favorable or unfavorable incident has happened. This report doesn't provide the actual evidence, yet it provides a logical explanation of the causes of the incident. A fact finding committee usually starts investigating the matter and uses this report as an indirect reference to the incident. It highlights different elements of the event which required further investigation or interrogation. In other words, we can say that it cannot be considered as a root cause analysis report but it can include as reference or unverified evidence of the incident.
